Classic Italian North Beach
Mario's food was perfectly prepared; the pasta al dente ; the main courses were excellent, too. Our guest had the lamb shank over polenta and raved about it.
The restaurant was recommended to me by someone who lives in San Francisco for 20 years. It did not disappoint. We had three people for dinner and each of us ordered a different entrée and each entrée was delicious. I would highly recommend this restaurant to others who want a good Italian meal.
Italian Family Run and Quaint
Caffe Macaroni was a great place to eat. We went there with a group of five and found the ambiance fun and quaint. The restaurant is snuggled into a very tight space so don't expect high luxury and spacious eating and new stylings. It was amazing to not be that way. The food was all very well prepared and with a real Italian feel and the service staff and owners were all so friendly and welcoming. We had the calamari, the black squid ink noodles, and a few of the specialty items that night and everyone was delighted with the appetizers and their meals. I highly recommend this quaint and friendly Restaurant.
We arrived 30 minutes early for our reservation. It was raining and thought I would have to wait outside. They prepared a table for us and got us in early (good start). Service was great, the food was outstanding. My family of 5 had a great time. Plenty of variety on the menu to please everyone. We loved the atmosphere - cozy and unique.

Special Italian eatery with friendly service and flare. Seating was very cozy in tiny room, but worth the quality food. Best if you can eat outside. Pasta and pizza a must. Deliciosa!
"Bottle of red....bottle of white" may have been written here. Small, classic, personable. First time diners, felt like family. Black pasta de mare excellent. Three entrees, one bottle wine, three desserts....138 with tip. You kidding me?? Highly recommended.
I have been eating and drinking here for 25 years and Mario always provides the freshest ingredients, well-prepared and authentically southern Italian (Naples), accompanied by very well chosen wines, all reasonably priced. It is our "Go To" spot in the City for virtually any occasion.

Favorite Italian Restaurant in SF
Nothing better than the owner greeting you and letting you know he's happy to have you. The food is great, the service is great, it's a little hole in the wall and that's fucking great. I love this place and will be eating here every time I visit San Francisco.
Tiny North Beach find
Excellent food and fun location for watching people walk by. The restaurant is tiny but has outdoor seating for nice days. The owners and staff were friendly and accommodating. By the end of the meal, you feel you are family!
So much fun. Classic San Francisco, great food, entertaining service. We sat outside on Columbus Enjoyed the Chicken Scallopini with Gnochi.
Still Fantastic after 20 years
Used to come often but have not been here since mid-90's!!! Food still over the top excellent, friendly competent staff - owner on premises. Best southern Italian I have had in my life. Thank you Mario for keeping it going all these years without losing a beat! My loss for letting it go so long without a visit.

We randomly saw this place on open table and decided to give it a try. While the dining area was small, we came on a weekday at 1100 and had no trouble getting seated. We started with the calamari and lentil soup. The calamari, which was served warm, was very fresh and complimented with beans and tomatoes. Not rubbery at all. The lentil soup was seasoned well and the soup base was clear broth, not very oily, which is a good thing. My husband had the risotto pescatore while I had the pasta amatraciana. Both were excellent. Lots of very fresh seafood in the risotto and my pasta was cooked perfectly al dente. Prices are reasonable as are portion sizes. We will definitely be returning!
A North Beach Legend
I've been going to Caffe' Macaroni for over 15 years, and I still look forward to having dinner there. Mario and his wife Stephanie do a fantastic job in making you feel comfortable and then serve some of the best Italian food there is. Mario always has such an extensive list of specials that I rarely order from the menu. Everything is good. Reservations for dinner are a must.
Favorite Italian Restaurant Anywhere
I love this place. A friend referred it a year ago - I've been three times on visits to San Francisco, and I'll continue to go back. I love this place.

As an almost-annual visitor to San Francisco, I make it my business to dine at Caffe Macaroni every time that I am in town. This time I brought colleagues to whom I had raved about it; they shared my enthusiasm after their visit. I travel to Rome most years and can compare Caffe Macaroni very favorably to small trattorie that you find in the historic center of the Eternal City.

Excellent dining experience
Situated in a small, unassuming but charming space, this restaurant manages to effortlessly deliver an exceptionally enjoyable dining experience. The menu is creative, the food is fabulous, the wine is excellent, and the service is attentive and personal. It is a restaurant to which we will return frequently, since it is extremely difficult to find better food, served in a more charming venue, by staff which is more friendly, unpretentious and capable. Our party tried a range of appetizers as well as entrees, including soup, fish and various types of pasta, and across the board the food was remarkably excellent. This restaurant is truly a gem.

Slightly Above Tourist Food
Maybe the restaurant had an off night but I just cannot imagine ever eating here again. The food was not bad but it was not good either. Not enough spices in the food. I just can't think of a reason to return. I certainly would never seek it out. Too many places in North Beach and the rest of the city that have just as good as and better Italian food PLUS a decent bar.
Caffe Macaroni is fantastic. Easily the best Italian food I've had in the US. Eat here.
Wanted to love it
I scoured all the Yelp reviews before making a reservation at Caffee Macaroni. We went with another couple - then the couple at the next table had an extra couple squeeze in with them - which was annoying - not only because the other man in our party couldn't hear well but also because it made for a tight fit. Food was only average at best. Lots of olive oil! Salmon was fairly tasteless.
Dinner was fabulous. Very small, intimate restaurant. Very authentic. We really liked this place.

We're always treated like family when we go to Macaroni, by the very friendly Italian family that runs this place. The ambiance is unique as well but the food is what keeps us coming back, and the menu is always fresh with specials of the day. Can't wait to go back!

The service staff is so friendly and attentive, even though they were so busy. I requested a downstairs inside window table and waited a bit since the place is pretty small. The calamari was tender and delicous. The arugula salad with fennel was perfectly dressed with balanced dressing. The pizza could have been baked just a tad longer to have browned crust. It is amazing that that tiny kitchen produces such great food at reasonable prices. We will be back!
A Italian Experience for my Birthday
This restaurant was the only place I wanted to go to for my birthday dinner with my friends. I lived in Italy for awhile and this place has not only the best Italian food I have had since coming back but a also a true Italian experience. Each of the four courses were small and delicious, served in a steady slow pace so we could talk and enjoy our meal together and the servers made us feel like we were at their home not their workplace. My Italian friends said that they expected to go home afterwards to their apartments in Florence. This restaurant is a must for anyone eating in the City.

Excellent food... Small restaurant yet cozy. Do not sit upstairs if you are over 5'9" due to the low ceilings- but worth it even if you have to duck 😉. Lots of specials off menu to choose from & friendly staff. They even held on to my scarf for me after i forgot it the other night! Authentic, old school foods. The dessert of the evening was spectacular!
